Summary:

The Patient Safety Manager is responsible for working collaboratively with leadership, medical staff, and hospital staff for the purpose of measurably improving the safety of patient care & clinical outcomes. The Manager analyzes data from multiple sources to identify opportunities for improvement, determine root causes, and implements actions needed for success. S/he is responsible for the reviews of serious adverse events & near misses and facilitates effective root cause analyses and corrective action plans to prevent recurrence. Supports leadership in developing a strong culture of safety and serves as a resource to others by coaching, mentoring and educating on patient safety topics and practices.

Responsibilities:

  1. Serves as hospital Patient Safety Officer and works collaboratively with Corporate Quality, Risk Management, Senior Leadership, and others to help implement the organization’s patient safety program and initiatives.

2.Responsible for timely reviews of serious adverse events & near misses. Facilitates effective root cause analyses and develops corrective action plans to prevent recurrence.

3.Ensures corrective actions plans are implemented and monitored in order in order to evaluate their effectiveness and ensure targeted changes are hardwired (sustained change over time).

4.Collaborates with staff, medical staff, and hospital leaders to ensure meaningful and actionable patient safety data & reports are in place and being used on a consistent basis to allow the organization to understand its current performance and where focused patient safety improvement efforts may be needed.

5.Uses the data-driven approach described above to identify the high priority patient safety improvement opportunities, helps develop workplans and engage teams necessary to improve patient safety, and provide follow-up & monitoring to evaluate progress and ensure process changes are hardwired.

6.Serves as a leader and resource for helping to create and sustain a strong culture of safety and high reliability across the organization.

7.Develops and delivers coaching, mentoring and education on patient safety to others on a regular basis.

  1. Chairs the organization’s Patient Safety committee, establishing agendas, membership and processes needed for continuous improvement of patient safety and clinical outcomes.

9.Work closely with other Nuvance Hospitals and organizations to share information across the organization in a timely manner to improve patient safety and improve clinical outcomes.

10.Participate in other patient safety duties and responsibilities working collaboratively with all others.

11.Maintain and Model Nuvance Health Values.

12.Demonstrates regular, reliable and predictable attendance.

13.Performs other duties as required.

Other information:

Education and Experience Requirements:

·Bachelor’s Degree in Nursing or a related clinical field required (or equivalent experience); Master’s Degree preferred.

·Three (3)+ years of experience in an acute care setting required, preferably in a leadership role.

·Minimum of one (1)+ year full time experience in acute care quality, patient safety, or risk management required.

License, Registration, or Certification Requirements:

·Current NYS license as a Registered Nurse or other clinical professional.

·Certified Professional in Patient Safety (CPPS) preferred.

·Certified in Lean, Lean Six-Sigma, or comparable methodology preferred.

Minimum Knowledge, Skills and Abilities Requirements:

·Experienced in the IHI Root Cause Analysis & Actions (RCA2), Model for Improvement, Lean-Six Sigma, or other formal process improvement methods. Able to coach and develop others on same.

·Strong organizational, problem solving and time management skills.

·Contemporary knowledge of patient safety and performance improvement methods in healthcare.

·Exceptional level of professionalism, discretion, and the ability to work on highly sensitive and confidential projects with a wide variety of others.

·Outstanding negotiation, written and verbal communication skills

·Outstanding listening, interpersonal relationship-building, and problem-solving skills

·Able to prioritize and manage multiple projects simultaneously with demonstrated ability to complete projects successfully on time and within budget.

·Excellent leadership attributes including positive, can-do attitude and the ability to lead, coach, mentor, and create a high-performing organization.
