Job Overview:

Supports Cancer Center practice physicians in all phases of patient care management as it relates to new patient referrals and the oncology nurse navigator program. Coordinates new patient referral distribution and establishes patient care management. Answer calls associated with new patients referrals and coordinate with physician/nurse practitioner schedule. Develops relationships with referring physician and office staff. Develops educational and informational marketing programs.

Qualifications:

Required:

  • Must be a graduate from an accredited school of nursing

  • Successful completion of all orientation programs

Preferred:

  • Oncology experience

Certifications and Licenses Required:

  • Current Registered Nurse License in the state of New Jersey

  • Basic Life Support (BLS)

  • ONS/OCN certification

Scheduling Requirements:

  • Full Time

  • Day shift

  • 37.5 hours weekly

Essential Functions:

Answer calls independently for select issues related to new patient consults and follow up visits. Assists in Cancer Center marketing of new patient referrals. Assists patients and families in obtaining financial counseling and appropriate referrals. Collaborates closely with physicians and members of the healthcare team to coordinate care of the patient. Eliminates barriers to timely diagnosis and treatment. Collaborates with psychosocial and community outreach department to educate referring physicians and facilities on available services. Communicates patient needs to appropriate healthcare team members and departments. Communicates with referring physician s offices as appropriate to meet patient and physician needs. Creates charts for new consults by obtaining all necessary information from various services including lab, pathology, radiology and physician offices. Develops and enhances processes to improve the clinical experience for patients. Streamlines patient access to care, enhances quality care, and increases patient and provider satisfaction. Develops educational programs, materials, documentation tools and informational seminars for patients and physicians. Educates and supports patients and their families.

In collaboration with staff physicians, provides clinical support and counseling to patients, families and members of the healthcare team. Inform the appropriate physician when the new patient consult has been scheduled. Personally greets patients upon arrival. Provides written educational materials about the program and their disease. Refers patient and families to appropriate physician or specialist for diagnosis and treatment. Screens and triages patient self-referral or physician office related phone calls. Provides timely physician appointments. Serves as a resource to the individual patient and referring physician throughout treatment. Updates and maintains statistics on new patient volume and referral patterns. Works closely with Research Dept. to facilitate patients ease in enrolling in clinical trials. Works in the community setting to promote oncology education, prevention and early detection through outreach programs and physician practice meetings.
